Cari-American Services, Inc. ensures the recipient meets the following eligibility criteria for Supported Independent Living (SIL) services.
- Recipient is receiving services from the Developmental Disabilities Program.
- Recipient is at least 18 years of age.
- Recipient desires to live in his/her own home in the Community.
- Recipient is unable to live in his/her own home in the Community without ongoing supports.
Assigned SIL coaches works directly with the recipient and the Medicaid Waiver Support Coordinator (WSC) as a team along with the recipient’s circle of supports to advocate, plan, and maintain the supported living situation. The circle of supports may include anyone the recipient chooses such as family, friends, providers, neighbors etc. Therefore, the role of the circle of supports is to work in conjunction with the recipient in planning for his/her life.
For recipients who may not clearly express their choices, Cari-American Services, Inc. will ensure that a close friend or guardian is involved in the interview and selection process. For those recipients requiring housing assistance, the team will explore a wide-array of supports and services, which are individualized to each recipient’s unique situation.
Respecting recipients’ personal rights is paramount. As such, recipients in SIL experience greater opportunity to maximize personal control of their home environment and daily routines by not occupying state or provider owned housing. Because everyone has the right to privacy, any home visit requires agreement of the recipient ahead of time and should be conducted with sensitivity, courtesy, respect, and restraint from unwarranted intrusion. Except for life-threatening emergencies, a recipient’s home should be entered only when the recipient extends an invitation or permission. Everyone enters as a guest because of the recipients freely given hospitality.
Thorough planning, detailed documentation, and careful analysis of the recipient’s progress are all factors contributing to meeting the needs and/or supports of the recipient. Support and services provided by the Agency will enable the recipient to demonstrate an increase in his/her abilities, self-sufficiency, and changes in their lives consistent with their Support Plan goal(s).
The agency will ensure that services are provided 24 hours a day, seven days a week to our SIL recipients. Assigned staff will be available to address recipient’s issues, which may arise at any given time. As such, a qualified designated/backup person is identified to provide supports in the event the on-call person is unavailable.
